  • Complete list of supplements and/or medications

  • Copies of any recent labs, imaging, or relevant doctor's reports (please do not drop these off at the office prior to your appointment - we do not have secure storage space to keep them)

  • Completed Intake Forms and Nutritional Assessment

What to Bring to an NAD Appointment

NAD infusions can take up to six hours. The average time is usually around four hours. For this reason, we ask that you bring a snack and something to drink as well as a book or something to entertain yourself.
